Antimicrobial Resistance Rates and Risk Factors for Extended-Spectrum Beta-Lactamase-Producing Escherichia coli-Associated Urinary Tract Infections in Older Outpatients in East Anatolia from 2011 - 2019

Background: Community-acquired urinary tract infection is among the most common infections in older adults. Regardless of age, frequently detected causative microorganism Escherichia coli. In parallel with increase antibiotic use, frequency community-acquired extended-spectrum beta-lactamase-producing E. coli (ESBL-E. coli) has reached critical levels. The use empirical therapy determined by assessing patient-based risk factors. Therefore, knowing factors and antimicrobial resistance can guide treatment to shape treatment. Objectives: This study aimed determine risks frequencies selection for ESBL-E. coli-associated (UTI) elderly patients. Methods: a retrospective cohort study. It was carried out between 2011 - 2019. growth ≥ 105 colony-forming units (cfu)/mL urine culture included 815 patients aged 65 over who applied outpatient clinics. Results: Two hundred sixty (31.9%) had coli, rates were highest (100%) penicillins + β-lactamase inhibitors. lowest carbapenems, aminoglycosides, phosphonic acid, nitrofurantoins. Risk ESBL-producing bacteria determined. These presence benign prostatic hypertrophy, last three months, history UTI year, catheter uses male gender, hospitalization year (P < 0.05). only independent factor which increased ESBL 2.8 times. Conclusions: Carbapenems be chosen as parenteral options, acids nitrofurantoin oral options treatment, especially past year.
